In addition to completing the application and your delineation of
privileges, we will need the following:
Copy of Governmental Issued Photo ID (CDL acceptable)
Current California License
DEA Certificate
Fluoroscopy Certificate/Laser Certificate (if applicable)
Malpractice Insurance Certificate
Curriculum Vitae

Please be sure to complete the application completely, and send current licenses, as an incomplete packet can delay your appointment. We would like to draw you attention to the by-laws which require your hospital admitting privileges be within the city limits of San Francisco or at Seton hospital, or that you bring an assistant whose admitting privileges meet this criteria. Processing will take 2-12 weeks. We have also included for your convenience, our scheduling criteria for your scheduler, as well as a patient information sheet to be copied to your patients.

**IF DICTATING LESS THAN 24 HOURS BEFORE THE SURGERY START TIME, PLEASE PRESS THE * KEY ON YOUR PUSH BUTTON PHONE AT ANY TIME DURING YOUR DICTATION TO MARK THE FILE AS "STAT".
  1. Enter the LOCATION CODE (70) followed by the pound key(#).
2. Enter the PHYSICIAN ID followed by the # key.
3. Enter one of the WORK TYPE numbers listed below followed by the # key:
  1# = Preoperative H &P
2# = Regular H&P
3# = Operative Report
4. Enter patient's MEDICAL RECORD NUMBER followed by the # key. If you do not have the medical record number, press 9 five times followed by the # key.
5. System will beep to prompt you to begin dictating:
  State your name
State/Spell your patients name
State the Date of Service

TO DISCONNECT FROM THE SYSTEM:
Press (5), note Job #. At the end of each dictation, the job number (confirmation number) for that dictation will be given to you.

TO DICTATE MULTIPLE REPORTS:
At end of first dictation, press (8), note Job #, follow steps 2-4 above.

TO REVIEW DICTATION:
Press (3) once or several times to incrementally rewind with auto playback. Press (2) to resume dictation.

PLEASE NOTE:
The system is voice activated; if you stop speaking or speak too low/softly, a tone will return until you begin speaking or you speak louder. If tone continues for more than 5 minutes, you will be disconnected.

TOUCH TONE FUNCTIONS:
  1 Stop
2 Record/Pause/Record
3 5 Second Incremental Rewind and Play
4 Fast Forward (press 2 to stop) and Play
5 Disconnect/Plays Job #
6 None
7 Rewind and Play
8 End Report, plays Job #, begin new report
